Literacy Intervention Intake Assessment
Best for: Children in Kindergarten through Grade 8 who are experiencing challenges with reading, spelling, writing, or literacy development.
If you've ever wondered why your child is struggling with reading or writing, this assessment is the first step toward finding answers.
Our Comprehensive Literacy Intake Assessment is completed collaboratively by Carly Axani, Registered Speech-Language Pathologist, and Dr. Sarah Novosel-Ulbrich, Literacy Specialist. Together, we identify your child's literacy strengths, pinpoint the underlying skills affecting their progress, and develop an individualized plan for success.
During the assessment, we evaluate the foundational skills that support literacy development, including:
- Phonological and phonemic awareness
- Phonics, decoding, and word reading
- Reading fluency and accuracy
- Reading comprehension
- Spelling (encoding)
- Written expression
- Vocabulary and morphology
- Oral language skills (when indicated)
Following the assessment, you'll receive:

✓ An explanation of why your child is experiencing difficulty
✓ Individualized recommendations based on the Science of Reading
✓ A personalized intervention plan designed specifically for your child
If intervention is recommended, we'll carefully match your child with one of our highly trained teachers, who will deliver individualized, evidence-informed instruction in your home while Carly and Sarah continue to monitor progress and adjust goals as needed.

Grade-Level Literacy Review
Best for: Children in Kindergarten through Grade 8 whose parents want to ensure they are developing age-appropriate literacy skills or monitor progress over time.
Not every child needs a comprehensive literacy assessment. Sometimes, parents simply want reassurance that their child is on track—or to identify small gaps before they become bigger challenges.
Our Grade-Level Literacy Check-In is a focused screening designed to compare your child's literacy skills with grade-level expectations. We'll identify areas of strength, highlight any skills that may benefit from additional support, and provide practical recommendations to help your child continue to grow with confidence.
During the assessment, we evaluate key literacy skills, including:
- Phonics and decoding
- Word reading accuracy
- Reading fluency
- Reading comprehension
- Spelling
- Written language
- Grade-level literacy expectations
Following the assessment, you'll receive:
✓ A summary of your child's literacy strengths
✓ Identification of any emerging areas of concern
✓ Practical recommendations to support literacy development at home and school
✓ Guidance regarding whether additional assessment or intervention is recommended
